Hospitals and clinics those providing Exclusive Siddha Treatments are eligible for membership. Before filling the application, please keep ready of the following documents (below 1mb).
- valid Clinical Establishment Act Registration Certificate (if not available) Local Registration Certificate.
- PAN card of the Organization or Owner If you have GST registration/ MSME Registration available Please upload.
- AADHAR card of the applicant.
- Passport size photo of the applicant.
- BSMS or BIM Degree certificate (if additional qualification is mentioned then upload PG and PhD Degree certificates).
- Registration Certificate (TNSMC/TNBIM/CCIM/NCISM in a single PDF).
- Recent picture of the Front Elevation view of the Hospital/Clinic/Center.
- Recent picture of the Entrance of Hospital/Clinic/Center.
- Recent picture of the Hospital/Clinic/Center Main Board in the entrance or door step. If any quality accreditations, then upload NABH Accreditation Certificate (If available), ISO Certificate (If available), Rohini Registration Certificate (If available).

New Registration for first time – Rs. 1000 (Non-Refundable) and Annual Renewal Membership – Rs. 500 per year (Non-Refundable) - Annual payment should be paid before Jan 31st every year.
